De Soto HS
10th & 12th Grade Registration (M - Z) for new and returning students.
Hours: 8am - Noon & 1pm - 3pm
Scoliosis screenings will be done on all 7th and 9th grade students. Dress appropriately. Girls need to wear swimsuit top or sports bra.
Jr. High and High School ONLY-Parents of new students to the school district are encouraged to stop by the main offices at the Jr. High and Sr. High on July 31 or August 1 to pick up an enrollment packet and fill out a release of information form. This will help expedite the enrollment process so that new students will be ready to enroll with their grade levels. Proof of residency is also required at this time.
Students must attend registration on their designated date. Students will not be allowed to register early.

*Proof of Residency (i.e. Utility Bill, Rental Contract, Real Estate Contract, Payroll Check)
*Immunizations *Transcripts *Attendance Record *Social Security Card *State Birth Certificate *Discipline Record/Letter Stating No Discipline *Full information of last school attended (address and phone number)
All Students
*Current orders from a physician for medications, restrictions, etc
*Any current immunization information
